Gay Health Action Questionnaire. States that completed forms can be returned to collection box: Phoenix Club or Quay CoOp or posted to: P.O. Box 97, Brian Boru Street. Provides information about GHA. Explains the purpose of the questionnaire which is to gain accurate information concerning the current standard of services in STD clinics. Notes that the general level of STD services in Ireland is very poor and the complaints received from doctors. Highlights the need to know specifically the major areas of concern. Provides a list of 15 questions with tick-the-box 'Yes/No' answers.
